Many insurance companies with HMO or POS plans will not pay for specialty practice
charges unless a referral for the appointment is given from the primary care physician
Please check with your insurance company by calling the customer service 800 number
on your insurance card to see if you need to have a referral prior to scheduling an
appointment with us. If your insurance company requires you to have a referral from
your PCP or family physician, you must obtain that referral prior to being seen by
If no referral is required, please call us to schedule an appointment. If a referral is
required, you must contact your PCP’s office. Ask them to fax a referral to our office at
(770) 619-3203 prior to your appointment date.
Managed care requirements make it necessary for patients who do not have their
referral at the time of the appointment to either reschedule their appointment or make
payment at the time of service.
In order to accommodate the needs & requests of our patients, we have tried to enroll
in numerous managed care insurance programs. While we are pleased to be able to
provide this service to you, it is impossible for us to keep track of all the individual
requirements of these plans. Each plan has different stipulations regarding its policies.
